STEP 1:  SUBMIT INFO TO RECEIVE AN EVALUATION AND BUDGET ESTIMATE:
	Health History Form for Bariatric Surgery COMPLETELY FILLED OUT
	Create Intro Letter about yourself at end of HHF along with questions, concerns and possible dates for your surgery.
	ANY recent (within last 6 months) Lab work or other exam results (EKG, Ultrasounds, etc) you have had in the last six (6) months whether for your evaluation for weight loss surgery or due to your co-morbidities.  After the surgeon reviews your HHF, he will specify other tests or exams he will need before you undergo surgery.
	Full body Pictures taken (clothed), and saved in following format:  jane_doe_pics.jpg.  If you are emailing the HHF and labwork, please save them in the same format:  jane_doe_labs1.pdf, etc…. before you email them to us.

STEP 2:  SUBMIT PAYMENT FOR EVALUATION PLAN AND BUDGET ESTIMATE:
	$50 USD non-refundable Evaluation Plan Fee sent via PayPal.  In 5-7 days you will have your ...
	Evaluation Plan and Budget Estimate received back.  This is an individualized evaluation of your ability to undergo surgery, what would be required pre-op, how long you will need to stay, the procedure/s best suited for you and their prices, as well as detailing your budget for hotel, aftercare, Medical Concierge accompaniment, etc.  Call us with ANY concerns or questions!

STEP 3:  IF YOU DO NOT HAVE A PASSPORT YET:
	Passport applied for or verified valid for travel (must be valid for six months past visa date/arrival date).  Check with your Post Office to see the wait time for new passport issues.

STEP 4:  OBTAIN NEEDED EXAMS PER SURGEON ON YOUR TREATMENT PLAN:
	Schedule still needed Pre-op testing that surgeon requires with your PCP,
	OR if you will do the additional testing in Brazil, request the price list from MNB for these exams.

STEP 5:  FORWARD RESULTS FROM TESTS TO MNB
	Send via email or fax, the results from obtained labs.

STEP 6:  RESERVE YOUR SURGERY DATE/S:
	Surgery preference dates submitted to Concierge@MedNetBrazil.com .
	$150 USD Surgery Booking Fee sent via PayPal to reserve surgery date

STEP 7:  BOOK YOUR AIRLINE TICKETS and APPLY FOR YOUR BRAZILIAN VISA
	Tickets purchased (you need to run your itinerary past MNB first!)
	Visa applied for with nearest Brazilian Consulate/Embassy.  You do not need to have tickets purchased, but you must have an itinerary ready for them.  There is currently a US$100 reciprocity fee to obtain a Brazilian visa.

STEP 8:  OBTAIN CLEARANCE FROM YOUR PCP:
	Surgical Risk Letter obtained from PCP and forwarded to MNB

STEP 9:  CONFIRM ALL YOUR TRAVEL AND STAY DETAILS THROUGH OUR CONCIERGE
	If you are having your pre-op tests done in BRAZIL, you marked the Concierge Summary Checklist with the tests you will need us to schedule for you once you arrive.  Send this form back so we can create your FINAL CONFIRMATION LETTER!!
	FINAL CONFIRMATION LETTER received and reviewed.  Please contact us immediately with any questions or concerns!

STEP 10:  PAY FOR YOUR SURGERY AND AFTERCARE PACKAGE:
	Pay for the Package Plan recommended by the surgeon and your Surgery Fees.  These must be paid in full no later than 2 weeks before your arrival.  You can pay via PayPal or safely and legally via wire transfer to our Bank of America business account for a small fee.
	Make sure MedNetBrazil has received a FACE picture of you for your airport pickup
	ALL PRE-OP TESTS, EKG, SURGICAL RISK FROM PCP, ETC, MUST BE BROUGHT WITH YOU IN HARD COPY FORM, EVEN IF YOU PREVIOUSLY SENT, SCANNED OR FAXED THEM!!!
