STEP 1:  SUBMIT INFO TO CREATE A TREATMENT PLAN AND YOUR TOTAL BUDGET:
	Intro Letter/Wish List of Procedures you feel you need
	Health History Form for Plastic Surgery COMPLETELY FILLED OUT
	Pictures sent, saved in proper MNB file format (jane_doe_pics_face.xxx)
	If available, recent (last 6 months) Blood work/lab results (the surgeon will detail additional labwork you will require in your individualized Treatment plan).

STEP 2:  SUBMIT PAYMENT FOR TREATMENT PLAN/BUDGET
	$100 USD non-refundable Treatment Plan Fee sent via PayPal.  If you book your surgery date within 3 months of your plan’s date, $100 will be rebated off your surgery fees!  Within approximately 5 business days you will have your…

STEP 3:  IF YOU DO NOT HAVE A PASSPORT YET:
	Passport applied for or verified valid for travel (must be valid for six months past visa date/arrival date).  Check with your Post Office to see the wait time for new passport issues.

STEP 4:  RESERVE YOUR SURGERY DATE/S:
	Surgery preference dates submitted to Concierge@MedNetBrazil.com We will tell you the date you need to ARRIVE in Brazil based on your surgery date.
	$100 USD Surgery Booking Fee sent via PayPal to reserve surgery date(s)

STEP 5:  BOOK YOUR AIRLINE TICKETS
	Tickets purchased (PLEASE run your itinerary by us first!   We coordinate your transportation, testing and consultations before you arrive and we must make sure it works!)

STEP 6:  APPLY FOR YOUR VISA FOR ENTRY INTO BRAZIL
	Visa applied for with nearest Brazilian Consulate/Embassy, US$100 reciprocity fee.

STEP 7:  OBTAIN NEEDED EXAMS PER SURGEON ON YOUR TREATMENT PLAN:
	Current Labs and EKG – should be done at 2 months prior (surgeon can make an exception) to surgery to be able to address any nutritional or deficiency problems before arriving.
	Any determined deficiencies being addressed with Primary Care Physician

STEP 8:  OBTAIN CLEARANCE FROM YOUR PCP:
	Surgical Risk Letter obtained from PCP and forwarded to MNB
	Duoderm Dressings ordered and received if recommended on Treatment Plan.

STEP 9:  CONFIRM ALL YOUR TRAVEL AND STAY DETAILS THROUGH OUR CONCIERGE
	Concierge Summary Form filled out and forwarded to MNB
	FINAL CONFIRMATION LETTER received.  Questions?  Clarifications??

STEP 10:  PAY FOR YOUR SURGERY AND AFTERCARE PACKAGE:
	Pay for the Medical Basics Package Plan recommended by the surgeon and your Surgery Fees.  These must be paid in full no later than 2 weeks before your arrival.  You can pay via PayPal or safely and legally via wire transfer to our Bank of America business account for a small fee.
	Make sure MedNetBrazil has received a FACE picture of you for your airport pickup
	ALL PRE-OP TESTS, EKG, SURGICAL RISK FROM PCP, ETC, MUST BE BROUGHT WITH YOU IN HARD COPY FORM, EVEN IF YOU PREVIOUSLY SENT, SCANNED OR FAXED THEM!!!
